Converted code blocks that had placeholder descriptions into tables. I've decided to leave code blocks, which only list placeholders as is for now. Those can be converted later with added descriptions, when a consistent format has been decided on.
I've unified the formatting in all of the remaining code blocks by removing unnecessary hyphens and spaces, resulting them being much cleaner and easier to read.
Also fixed 2 broken links in the index section and updated the MiniPlaceholders v3 info admonition.
